HOW IT WORKS
A few steps before your first visit.
01.
Share your information.
Have your insurance card, photo identification and the name of the service you’re interested in ready. Our staff will explain how to submit your information through our designated intake process.
02
Let us review your coverage
Our team will review your plan and any referral or authorization requirements that may apply to your requested service.
03
Understand your next steps
We’ll discuss the information available about your coverage and any additional steps needed before scheduling. Your insurance provider can confirm your benefits and financial responsibility.
INSURANCE BASICS
Know what to ask before your appointment.
Even when a service is covered, you may have an amount to pay. These terms can help you understand the information provided by your insurer.
01.
Copay
A set amount your plan may require you to pay for a covered service.
02.
Deductible
The amount you may need to pay for certain covered services before your plan begins sharing those costs.
03.
Coinsurance
Your percentage of the cost of a covered service, as determined by your plan.
04.
Prior authorization
Approval your insurer may require before certain services or treatments. Authorization does not guarantee payment.
