Your results are part of a broader conversation about your health. The provider reviewing your testing can explain what the findings mean in relation to your symptoms, medical history and care plan.
Ask whether a follow-up appointment is needed. If you have not received your results within the timeframe you were given, contact the office for guidance.

Contact our office before arriving to confirm what your particular test requires.
Follow the instructions provided for your specific test. Some tests require fasting, while others do not. Do not stop or change your medication unless your healthcare provider instructs you to do so.

Our office is wheelchair accessible. Please contact us before your visit if you have accessibility needs or questions about arriving at the building.
INSURANCE & PAYMENT
Schedule your appointment
Coverage and out-of-pocket costs depend on your insurance plan and the testing requested. Our staff will review your insurance information and help you understand the next steps before scheduling.
Ask whether your visit involves any separate laboratory charges and how to confirm those costs. We also accept HSA and FSA payments.
Insurance acceptance does not guarantee coverage for every test or service.

Provide your patient information, identification and insurance details through our designated intake process. Our staff will verify your coverage and arrange for you to receive the required consent forms.

Once your information has been verified and the required consents are signed, our staff will contact you to schedule. Confirm any preparation instructions before your visit.
