Provide your patient information, identification and insurance details through our designated intake process. Our staff will verify your coverage and arrange for you to receive the required consent forms.

Schedule your appointment

Once your information has been verified and the required consents are signed, our staff will contact you to schedule your initial visit.

YOUR FIRST APPOINTMENT

Start with your history. Make room for your goals.

Your initial visit is an opportunity to talk about what you are experiencing and how it affects your daily life. Your provider will review relevant health information and discuss possible treatment options with you.

  • A list of current medications, including doses.

  • Any vitamins, supplements and over-the-counter medications you take.

  • Information about medication allergies or past side effects.

  • Previous mental health treatment details, if available.

  • Questions or concerns you want to discuss.

You do not need to have every answer before seeking care. Share what you know and let your provider guide the conversation.
