Start with the reason you’re seeking care
When requesting an appointment, let our team know which service you’re interested in or give a brief description of the support you’re looking for. You don’t need to know the medical name for your concern.
You might be seeking therapy, psychiatric medication management or primary care. If you’re unsure where to begin, ask our team which appointment would be appropriate.
Before your visit, confirm the appointment location, time and any instructions specific to your service.
Help us get to know your health history
Your first appointment is an opportunity to discuss what brought you in and provide background that may be relevant to your care.
Depending on the service, the conversation may include your current concerns, previous treatment, medications and health history. You can also share what has worked well for you in the past and what you would like to approach differently.
You don’t need to tell your story perfectly or remember every detail. Bring any information you have and let your provider know when something is unclear.
Tell us what matters to you
Your goals are an important part of the conversation. Think about what you hope to understand or address through care.
Perhaps you want help making sense of a concern, discussing treatment options or establishing a more consistent approach to your health. You may also have preferences or worries about a particular treatment.
Share those thoughts with your provider. Questions such as “Why are you recommending this?” and “What other options can we discuss?” can help you participate in decisions about your care.
Understand your next steps
What happens after your first visit will depend on the service, your needs and your provider’s assessment. Your appointment may lead to further evaluation, a discussion of treatment options or a plan for follow-up care.
Before leaving, make sure you understand what has been recommended and whether you need to schedule another appointment. If testing is discussed, ask how results will be shared.
