Looking for a primary care provider?

You’ve probably experienced it before: you leave an appointment and remember a question you meant to ask. Between a busy schedule and the details discussed during your visit, it can be easy to forget something that matters to you.

A little preparation can help you feel more confident about the conversation. Whether you’re scheduling a routine visit or discussing a new concern, here are a few topics to bring to your next primary care appointment.

Start with what matters most to you

Write down your questions before your visit and identify the concern you most want to address. If you have symptoms, note when they began, how often they happen and what seems to make them better or worse.

You don’t need medical terminology. Describing what you’ve noticed in your own words gives your provider a place to begin. MedlinePlus offers more guidance on preparing for a conversation with your provider.

Review your medications and supplements

Bring an updated list of prescription medications, over-the-counter products, vitamins and supplements, along with any known allergies.

Mention side effects, difficulty remembering doses or concerns about cost. These details belong in the conversation, even if a different provider prescribed the medication. See MedlinePlus’s appointment preparation guidance.

Ask about preventive care

Your appointment is also an opportunity to ask about looking after your health over time. Questions you might bring include:

  • Which screenings or vaccinations should we discuss?

  • Does my family health history affect what you recommend?

  • Am I due for any follow-up care?

  • Are there health habits you suggest I focus on first?

Use these questions to start an individual discussion rather than arriving with a fixed list of tests you expect to receive.

Make room for everyday concerns

Think about what you would like your provider to understand about your daily life. Perhaps your work schedule makes appointments difficult, you’re concerned about paying for care or you’ve been meaning to bring up changes in your sleep or mood.

You can begin simply: “There’s something else I’d like to discuss.” You don’t need a perfectly prepared explanation.

Your appointment checklist

Before you leave home, gather:

  • Your questions and notes about symptoms

  • Your medication, supplement and allergy list

  • Relevant medical records or recent results, if available

  • Your identification and insurance information, if applicable

  • Any forms the practice has requested

Before leaving the appointment, ask how you’ll receive results, what the next steps are and whom to contact with questions. Ask for clarification or written instructions when needed. MedlinePlus explains ways to stay involved in your care.
